Program Guidelines
The Lake Meade Fire & Rescue (LMF&R) Ambulance Membership program is available to any resident who resides within the designated "primary response area" for LMF&R. Any individual residing outside this area will be referred to the appropriate adjoining Fire Department and/or EMS Service.
Annual memberships to the LMF&R Ambulance Membership Program are $20 per household. This fee covers all residents of the household and provides for emergency ambulance transportation only.
LMF&R does not engage in "routine", or non-emergency transportation.
When possible, and within LMF&R’s discretion, LMF&R will transport patients to the hospital of their choice, so long as that facility is within a 50-mile radius to Lake Meade. However, when necessary and required, the closest, most appropriate medical facility will be chosen as the patient’s destination.
ALL patients of the LMF&R Ambulance are invoiced for services provided. However, your LMF&R membership fees cover your co-payments, deductibles and other "out-of-pocket" expenses, where permitted by law.
LMF&R reserves the right to bill all third party insurers for services provided.
Those patients who are not members of LMF&R’s Ambulance Membership Program are responsible for the entire balance, including the portion remaining after receipt of insurance payments.
Subscriptions are renewed annually, and cover the period from April 1 through March 31 of the ensuing year.
Should LMF&R’s Ambulance be unable to render immediate assistance, LMF&R has entered into reciprocal agreements with both East Berlin and York Springs. These companies have agreed to honor memberships to LMF&R as their own. Similarly, LMF&R will honor memberships to their respective programs.
LMF&R is not responsible for any charges to a patient by any other entity.
Current LMF&R fees for ambulance services include:
Base Rate: $475.00
Mileage: $11.00
Cervical Collars: $10.00
Oxygen: $25.00
Disposable Supplies: $10.00
AED Pads: $75.00
LMF&R reserves the right to use external collection entities to collect fees from those patients whose accounts become 90+ days delinquent, or who refuse to provide the necessary insurance carrier information for 3rd party billing.
This LMF&R membership program is not a contract for the provision of ambulance services. This is not a solicitation for the offer or sale of an insurance product. The terms and provisions of this EMS membership/subscription program are subject to change without prior notice. Medicare beneficiaries may be billed for co-payments or deductibles if required by law. All memberships/subscriptions are subject to acceptance by LMF&R and may be cancelled or revoked in LMF&R’s sole discretion. The member agrees to remit any third party insurance payments received directly by the member to LMF&R.
